Keep the other copy for your records.

Be sure to fill in the form and send a copy with your Polar Heart Rate Monitor, our mailing address is on the bottom of the form. We will contact you by email on the repair costs.
To EXPEDITE repairs, you may authorize up to $75.00 with a credit card number on your repair form.

Please pack your watch & transmitter in a padded envelope along with your
completed repair form & insure the package. We are not responsible for
lost or damaged packages without insurance.
